Thank you for choosing us for your Orthopedic and Foot & Ankle problems.  My staff and I will do our best to assure that you have a pleasant experience during your visit.  This website has links which will allow you to download forms that will expedite the process when you arrive for your appointment.  Additionally there are some patient educational materials available as well as links to other useful sites that you may find of interest.

Please remember to bring with you the following:

1. Your insurance cards
2. A list of your current medications
3. A copy of any imaging study you have had done, such as an x-ray, MRI, CT scan, etc., especially if it was done recently.
4. A copy of any other doctor’s report on your problem, not all notes are necessary, however reports of any treatment you have received, such as surgery, are useful.
5. All completed registration paperwork if you downloaded it prior to your appointment.

Please try to arrive for your appointment approximately 15 minutes early for registration.  Every effort will be made to see our patients that their scheduled time and your cooperation is appreciated.  If you need to cancel or reschedule, please call our office at least 24 hours in advance.
